What does the secretary ask when she answers the phone?

Im needing to call the vet for my rat who has a rash on his neck and i want to kn ow what to say when i call the vet. is is pretty much like: 'hi i'd like to make an appointment' then what day then like with which vet and so on... Please give me full detail im a little nervous cause ive never dont this before! Thanks in advance
